Please complete the form below and upload your seller’s permit and state resale certificate so we can set up your business as tax-exempt. State tax laws require us to collect sales tax in the states listed below. If your business is located in one of these states and you have a valid seller’s permit, please submit the form along with the required documents.
Once your documentation is verified, we will update your account to reflect tax-exempt status for all future orders. You will receive an email notification when your submission has been reviewed and approved.
Please note: We cannot refund sales tax for orders that have already been placed.
Please submit resale certificates only for the states in which your company conducts business. We have provided each state's required resale certificate in the list below. If your state is not listed, you do not need to submit a resale certificate or seller’s permit to qualify for tax exemption.
Florida: No downloadable form available. Please submit a copy of your Annual Resale Certificate using our online submission form below.
New Mexico: Please submit a copy of your state-issued Nontaxable Transaction Certificate (NTTC) using our online submission form below, or download and fill out the Border States Uniform Sale for Resale Certificate.
Maine: Please submit a copy of your valid Resale Certificate issued by MRS using our online submission form below, or download and fill out the Uniform Sales & Use Tax Resale Certificate Multi-jurisdiction form.
Washington: Please submit a copy of your state-issued Reseller Permit using our online submission form.
Note: In the vendor field on your resale form, be sure to use "Earthwise Packaging Inc."
